Forms of Identification

Driver's License/Photo ID

If someone does not have their driver's license with them, they cannot test unless:

• s/he is from in town and can go and get their license immediately.
  OR
• s/he is from out of town without their driver's license at the time of a CPT test, and are able to return within two days with their driver's license. If the student should fail to return within the two days they will have to take the test again. No scores are processed without a driver's license. Students will have to wait two weeks before they can take their first test again, and then another two weeks for any retests.

Alternative Forms of ID

A current driver's license is our first choice for a photo ID. If there is no driver's license, then other accepted forms of ID are:

1. Another photo ID that is issued by a school, employer, or city/state/federal government agency.
2. Photo copy of photo ID is acceptable if the student is plainly recognizable.
3. Recognizable photo in a publication within last two years, with student's first and last name in the caption.
4. Personally recognized by a member of the testing staff.
5. ID letter on original high school/college letterhead. Letter must include: name, height, weight, race, hair description, and eye color, and student's signature, signed in ink in the presence of a school official who must NOT be a relative. Official must also sign the letter. The student must sign the letter again in the presence of testing staff so that a comparison may be made.
