Form of Identification for CPT

1. Current Driver's License or Current State ID or

2. Alternate Form of ID:

  • Current SWOSU ID
  • Another current photo ID that is issued by a city/state/federal government agency (such as tribal ID issued within the last two years).
  • Recognizable individual photo (not a group photo) in a publication within last two years, with student's first and last name in the caption.
  • Personally recognized by a member of the testing staff.
  • ID letter on original high school/college letterhead. Letter must include: name, height, weight, race,hair description, and eye color, and student's signature, signed in ink in the presence of a school official who must NOT be a relative. Official must also sign the letter. The student must sign the letter again in the presence of testing staff so that a comparison may be made.
  • Certificate of Indian Blood must be on tribal letterhead and include a recognizable recent photo with embossed seal or official’s signature overlapping a portion of the photo (your name and physical description including age, gender, height, weight, race, and hair and eye color can replace the photo); must include embossed tribal seal; and must be dated and signed by the tribal executive director.

Forms of Identification for CLEP

Primary ID and Secondary ID

Candidates must show two forms of personal identification.

1. Current Primary ID

Must be a government-issued photo ID. The ID must include the candidate's name, signature, and a recognizable photograph. Acceptable forms of primary ID include:

  • Current driver's license
  • Current state or Province ID card
  • Current passport
  • National ID
  • Tribal ID issued within the last 2 years
  • Naturalization card or certificate of citizenship

If the student is a minor and unable to provide a primary government issued ID, the the following form may be used for the primary ID:

2. Current Secondary ID

Must contain either a photo or a signature.

  • School ID
  • Military ID (Common Access Card)
  • Unused ID from the primary choices above

Forms of Identification for Keyboarding

Primary ID and Secondary ID

Candidates must show two forms of personal identification.

1. Current Primary ID

Must be a government-issued photo ID. The ID must include the candidate's name, signature, and a recognizable photograph. Acceptable forms of primary ID include:

  • Current driver's license
  • Current state or federal ID card
  • Current passport
  • Military ID with photo and signature
  • Tribal ID issued within the last 2 years
  • Permanent Resident Visa/Green Card

2. Current Secondary ID

  • Credit Card
  • Library Card
  • SWOSU ID
  • Utility bill (name and address must match the same information on the Primary ID)

Note: It is not required for the secondary form of ID to have a visible signature.

Form of Identification for GED

  • Current Driver’s License
  • Tribal ID issued within the last 2 years
  • Current passport
  • Military IDs
  • Other forms of government-issued (national or foreign) ID that show name, address, date of birth, signature, and photograph
  • Current ID provided by a postsecondary educational institution, with candidates name, address, date of birth, signature, and photograph
  • Certificate of Indian Blood (see Forms of Identification for CPT 2.f. above)
