Dental Insurance

The Oklahoma State and Education Employees Group Insurance Board (OSEEGIB) provides two dental plans to each SWOSU eligible employee at the employee's expense. The dental plans include the State Dental Plan (HealthChoice Dental) which is an indemnity plan and one pre-paid managed care dental plans (Fortis DMO). Each plan differs in benefits, copays, coinsurance costs, and maximum annual benefit amounts. The eligible employee must enroll in dental coverage in order to cover dependents. Limitations may apply for late enrollees. For additional details, individual plans should be consulted.

Fortis DMO

• Employee must select a primary care dentist at the time of enrollment who is a Fortis DMO.
