Oklahoma State and Education Employees Group Insurance Board (OSEEGIB) provides the health insurance program for SWOSU. Each eligible employee may choose between two State indemnity plans (HealthChoice High, and HealthChoice Basic) or two available Health Maintenance Organizations (HMOs) eligible in the area where the employee lives or works. Employee and any dependent coverage become effective on the first day of the month following the date of employment. If one dependent is covered, all eligible dependents must be covered, unless other group insurance verification is provided. SWOSU pays the premium for the eligible employee; dependent coverage is optional at the employee's expense. Health Maintenance Organization (HMO) Information

The employee must live or work in an eligible zip code area if an HMO is chosen for health coverage. All family members must enroll in the same health plan. With an HMO, there are no deductibles or claims to file. A Primary Care Physician (PCP) must be selected for each family member or one PCP for the entire family. Your PCP is your health care manager. It is important to always consult with your PCP for your medical care.
