Forms of Identification for ACT (National and Residual)

1. Acceptable

a. Current (not expired) official photo ID: Must be issued by your school or city/state/federal government (such as driver's license or passport) on which both your name and photo appear. If there is no expiration date, the issue date should be within the last two years.

b. Recent published individual photo: Recognizable individual photograph of you in a recent (within the last two years) publication, such as a newspaper or school yearbook, with your first and last name in the caption. Group photos are not acceptable.

c. School letter or transcript: Letters must meet all of the requirements below.

  • The letter must be on school letterhead and include, an attached recognizable recent photo with school seal or official's signature overlapping a portion of the photo or your name and physical description including age, gender, height, weight, race, and hair and eye color.
  • Form letters reproduced on school letterhead and then individually completed and signed in ink are acceptable.
  • All school-issued letters of identification and transcripts presented as ID must have a raised or inked school seal.
  • Transcripts may be used only if they include a recent photo as described above and are signed as described below.
  • You must sign the letter or transcript in ink in the presence of the school official (who may not be a relative), and that official also must personally sign the letter or transcript in ink. Printed, stamped, or photocopied signatures are not acceptable.

d. Notarized statement with photo: If you do not have acceptable photo ID or your school does not have letterhead stationery, contact a notary public (who must not be a relative). Attach a current photo of yourself to a sworn statement that includes your name. You must sign this statement in ink in the presence of the notary public, who must affix the notary seal or stamp to a portion of the photo.

2. Unacceptable

a. ACT admission ticket
b. Birth certificate
c. Charge, bank, check cashing, or credit cards with or without photo
d. ChildFind ID card
e. Fishing or hunting license
f. Graduation picture or family portrait
g. ID issued by an employer or organization membership card
h. Learner's driving permit, if it doesn't include a photo
i. Passport or other photo so old that the person presenting it cannot be identified
j. Phone calls to the test center to identify the examinee
k. Photo ID issued by a business for promotional purposes (e.g., amusement parks)
l. Photo ID of parents
m. Photo with student's name embossed or printed on the photo by a photographer
n. Police report of a stolen purse or wallet
o. Report card or diploma
p. Social Security card
q. Traffic ticket, even with a physical description and signature
r. Web page with photo

Form of Identification for CPT

1. Current Driver's License or Current State ID or

2. Alternate Form of ID:

a. Current SWOSU ID
b. Another current photo ID that is issued by a city/state/federal government agency (such as tribal ID issued within the last two years).
c. Recognizable individual photo (not a group photo) in a publication within last two years, with student's first and last name in the caption.
d. Personally recognized by a member of the testing staff.
e. ID letter on original high school/college letterhead. Letter must include: name, height, weight, race,hair description, and eye color, and student's signature, signed in ink in the presence of a school official who must NOT be a relative. Official must also sign the letter. The student must sign the letter again in the presence of testing staff so that a comparison may be made.
f. Certificate of Indian Blood must be on tribal letterhead and include a recognizable recent photo with embossed seal or official’s signature overlapping a portion of the photo (your name and physical description including age, gender, height, weight, race, and hair and eye color can replace the photo); must include embossed tribal seal; and must be dated and signed by the tribal executive director.

Forms of Identification for CLEP & Keyboarding

Primary ID and Secondary ID

Candidates must show two forms of personal identification.

1. Current Primary ID

Must be a government-issued photo ID. The ID must include the candidate's name, signature, and a recognizable photograph. Acceptable forms of primary ID include:

a. Current driver's license
b. Current state or federal ID card
c. Current passport
d. Military ID with photo and signature
e. Tribal ID issued within the last 2 years
f. Naturalization card or certificate of citizenship

2. Current Secondary ID

a. Employee photo ID
b. School ID
c. Social Security card
d. Military ID with signature only
e. Unused ID from the primary choices above

Form of Identification for GED

a. Current Driver’s License
b. Tribal ID issued within the last 2 years
c. Current passport
d. Military IDs
e. Other forms of government-issued (national or foreign) ID that show name, address, date of birth, signature, and photograph
f. Current ID provided by a postsecondary educational institution, with candidates name, address, date of birth, signature, and photograph
g. Certificate of Indian Blood (see Forms of Identification for CPT 2.f. above)
