8 weeks prior to desired internship.
Malpractice Insurance
Students must show evidence of coverage of malpractice/liability coverage prior to beginning the internship. A group rate of $15.00 is available through the division of Allied Health with Marsh Affinity Group Services of Seabury and Smith. Coverage is from September to September. Insurance payment should be made by August 30. This fee is part of program fees for the Health Information Management Program. Students in other majors should pay this fee to the Health Care Administration Program Director.

Procedure
- Obtain internship packet from faculty coordinator.
- Choose recommended site(s)/date(s)
- Complete internship planning form and make initial contact with site. Turn in planning form to faculty coordinator by deadline date.
- Turn in malpractice fee.
- Process enrollment for the appropriate semester and number of weeks.
- Wait for confirmation by faculty coordinator.
- Read and be familiar with objectives, responsibilities, and notebook requirements.
- Complete internship for times and dates assigned.
- Complete internship notebook and take to internship coordinator for review.
- Pick up notebook from site and turn in to faculty coordinator within 30 days of completion of internship.
- Write thank you letter to internship site.
- Pick up graded notebook from faculty coordinator.
Objectives
The facility coordinator will schedule the student for orientation and observation in departments or services within the health care site. Possible departments to visit include:

Responsibilities
Clinical Facility
Faculty Coordinator
- Coordinate the schedule for the assigned student.
- Receive the student’s complete report (notebook), evaluate, and send a letter evaluating the student’s performance. Factors to consider include completion of schedule, ability to deal with personnel, and submitted report in line with objectives. The student will be responsible for making arrangements for the return of the notebook to SWOSU.
Administrative/Departmental personnel working with the student
- Discuss information with the student relative to the objectives.
- Allow the student to collect samples of forms that do not contain patient identifiable information
Student
- Follow the schedule prepared by the clinical facility.
- Exhibit professionalism in arriving on time, dress, adhering to agency policies, attitude and in relating to all agency personnel.
- Handle all information, oral or written, in a confidential manner.
- Prepare a report (notebook format) containing the information outline in the objectives that meets the grade scale and requirements for the internship.
- Submit your report to your facility coordinator. You must then make arrangements for the notebook to be taken to SWOSU to meet the deadline outlined in the policy.
For each department or service area visited, the student should:
- document sufficient information to explain the subject area or identify the role of the department in providing patient care and support services.
- collect samples in the forms of documentation generated in the line of patient reports and/or administrative documentation.
- participate in meetings, committees, or other organized means of communication when appropriate and can be scheduled.
- outline the internal and external disaster plan.
