Practice Department Requirements

  1. You must be registered as an intern with the Oklahoma State Board of Pharmacy.
  2. All students on rotations must have a SWOSU name tag and a SWOSU College of pharmacy patch on their white coat. Starting Spring 06 all students must have a valid SWOSU picture ID.
  3. Professional Liability coverage must be in effect throughout your practice rotations. This coverage is obtained through the college of pharmacy.  You must be able to provide proof of current coverage if requested. 
  4. Criminal background checks will be performed by each student upon admission to the college of pharmacy.  A copy of this background check must be provided, if requested, to the Pharmacy Practice office or rotation site.  If the sexual offender box was not checked on the original form or if the student has had a break in enrollment a new background check must be performed and provided to the pharmacy practice office by the current semester date.
  5. All students will have a urine drug screen, at their expense, upon admission to the college of pharmacy.  Students must be able to provide documentation of this test if requested by the Pharmacy Practice department or a rotation site. If students do not have a copy of their drug screen or if enrollment has been broken, the student will be required obtain another urine drug screen, at their expense, and provide a copy to the Pharmacy Practice Department by the current semester date.
  6. CPR certification by the American Heart Association at the BLS level is required for rotations.  Students will be certified in PharmCare Lab V prior to starting rotations, this certification will last two years.  If the student has a break in enrollment and their certification will expire prior to starting or during their rotations they must obtain re-certification and provide documentation to the Pharmacy Practice office by the current semester date.
  7. Students are entirely responsible for ensuring completion of all forms pictured with an asterisks, providing necessary documentation and returning all required materials to the Pharmacy Practice office in Oklahoma City.
  8. Any student who has not returned their completed required forms by the current semester date will not be allowed on practice rotations.
  9. All students MUST have an E-mail address capable of accepting/sending attachments. Your E-mail address should be checked regularly. In addition, students are responsible for regularly checking their SWOSU provided e-mail.  Some information will only be sent via the SWOSU e-mail system. Students will be held responsible for information sent to any e-mail address provided by the student to the Pharmacy Practice office, and their SWOSU provided e-mail.
  10. The student is responsible to update the Pharmacy Practice office with any change in contact information.  This must be done within 10 days of a change. Submit change to Cassie Larman.
  11. Students are responsible to be familiar with the SWOSU Pharmacy Practice appearance policy, and to make sure that they are in compliance. .
  12. Any student who must be terminated because of conduct during the rotation MAY be given a grade of "F" and MAY not be reassigned until the next rotation, or the next semester, depending on the offense.
  13. Any student who is under investigation for actions occurring at a rotation site that could lead to possible disciplinary action will receive an incomplete for that rotation until the investigation is resolved. This will be enforced even if the student had been informed of a grade before the investigation began. Depending on the outcome of the investigation the "I" will be replaced with the earned grade or disciplinary action will be carried out.
  14. Students will obtain advance approval for planned absences, for example job interviews, from both their preceptor and faculty advisor. The student must make up the time absent before each rotation is considered complete. Students must arrange with preceptors to make up such hours.
  15. In the event of an unplanned absence, for example illness or car trouble, the student must notify both the preceptor and faculty advisor in a timely manner. Documentation of the event may be required. The student must make up time absent before each rotation is considered complete. A student's failure to do any of the above may result in an unexcused absence that can result in failure of the rotation.
  16. Cheating on any project or examination given in practice rotations (including plagiarism) will not be tolerated. Cheating will result in the grade of F for the rotation and dismissal from the course.
  17. Information you gain about a patient or site through your Pharmacy Practice activities must be considered personal and confidential. Such information must not be circulated or discussed outside the activities of your Pharmacy Practice settings. Discussions of patient care (example - Medical History, Medications, Disease States, etc.) or site information (example - financial information, pricing, customer lists, contracts, trade secrets, etc.) should not take place outside the site or in hallways, lunchrooms, or in the presence of patients or other departments and/or employees. Patient and site information should not be removed from the Pharmacy Practice site. Breech of trust through lack of confidentially can have serious consequences, not only for the patient and his family but for the Pharmacy Practice site as well.
  18. All reports to the Oklahoma State Board of Pharmacy will be made by the Pharmacy Practice Office. You will receive 220 concurrent intern hours for labs completed on the Weatherford campus. Each of the eight (8) practice rotations will be worth 160 concurrent hours, unless otherwise indicated by your preceptors.
  19. It is the students responsibility to unsure a preceptor is on duty while on rotations. If a preceptor is not on duty a student may work as a support person only. The other option is to leave the rotation site and make-up the hours when a preceptor is on duty. If either situation occurs, please contact your faculty advisor.
  20. No outside employment is permitted during pharmacy practice rotations unless a written request is made to the pharmacy practice department and approved by them.
  21. Failure to follow any of the above Pharmacy Practice Requirements can result in the student being removed from rotations. Students MAY not be reassigned until the next rotation, or the next semester, depending on availability
  22. At the end of each rotation you will complete a Student Evaluation of Experiential Experience, which will be provided by your preceptor. This evaluation should be turned in to your faculty advisor or the Experiential Program Coordinator (address below) as soon after your rotation as possible.

      5. Follow the on-screen instructions
    • Each rotation site will be available two (2) days before the rotation ends till the end of the semester. You may complete each evaluation only one time. Results will be tabulated after the class is over. If you have an evaluation site that is incorrect, please contact Dr. Christy Cox or Jan Noble in assessment.
