Practice Area: Rehabilitation Unit (2A, 2B, and & 2C)

Student responsibilities/activities include but are not limited to the following:

  1. Attend Rehabilitation Staffing Tuesday-Friday.
  2. Review assigned patients charts (H&P, admit history, medications, laboratory data, procedures, etc.) before each staffing.
  3. Obtain and record medication histories (current and previous), prescription or OTC (including drug, dose, schedule, route of administration, ADR’s, monitoring parameters and efficacy), food and drug allergies, and any special diet restrictions.
  4. Formulate individualized drug regimens based on desired outcomes, concurrent disease states and drug therapies. Recommend modifications in drug therapy when appropriate. (Alwaysreview recommendations with preceptor prior to discussing with the other members of the health-care team.)
  5. Review and monitor patient’s drug therapy for potential drug-drug, drug-disease, drug-diet, drug-lab test interactions, and recommend modifications in drug therapy when appropriate. (Always review recommendations with preceptor prior to discussing with the other members of the health-care team members.)
  6. Provide concise, applicable, and timely drug information to patients and/or the health-care team (doctors, physical therapists, occupational therapists, social workers, pharmacists, nurses, etc.), Always review recommendations with the preceptor prior to discussing with the team.
  7. Review Medication Administration Records ( MAR ’s).
  8. Present a journal club over a recent article/publication (Therapy and/or Disease State).
  9. Research and respond to drug information questions (as needed) to the patients and members of the health-care team.
  10. Present 1-2 patient cases as described in the Pharmacy Practice Manual.
  11. Discuss assigned topics (~ one/week); presenting on Fridays
  12. Attend and participate in other activities as assigned (i.e., Mercy CME department or Pharmacy Department)

Attendance:

Students are expected to be at the rotation site from 8:30 a.m. to 4:30 p.m. Punctuality for meeting on the units to review charts, scheduled meetings, and presentations is required. If a student must miss an event or be late, the preceptor should be notified in advance.

Appearance:

All students are expected to dress in a professional manner and wear a white lab coat and appropriate nametag at all times while in the hospital and in patient care areas.

Evaluations:

Students will be evaluated based on the goals and activities outlined above and in the Pharmacy Practice Manual. A midpoint evaluation will be given at the end of the second week and the final evaluation will be given at the end of the rotation month.

During the midpoint and final evaluations, the student will suggest changes in the rotation format. In addition, if at any point during the rotation the student has a suggestion, comment, and/or complaint, he/she should inform the preceptor immediately.

Grading and Evaluations:

An oral or written exam may or may not be given at the end of the rotation. The final grade will be assigned based on the midpoint and final evaluations.
