Requesting a Transcript

If you are unable to come into the registrar's office and sign for a copy of your transcript,
we need a written request.

Please include the following:

  • Full Name (Name attended under and your current name).
  • Date(s) of Attendance
  • Social Security Number
  • Date of Birth
  • Full address(es) where transcripts are to be sent AND your return address
  • Official or Unofficial copy (we cannot fax official transcripts)

The request must bear your signature!
