Job Description

Seeking anticoagulation pharmacist specialist for an Anticoagulation Clinic. Pharmacy Directed Anticoagulation Program - Responsible for ordering medication and pertinent labs and adjusting anticoagulation medication accordingly.


  • Progressive healthcare system
  • Team approach to healthcare delivery whereby the pharmacist will initiate, modify or continue drug therapy for a specific patient
  • Prescriptive authority
  • Preceptor for student and resident teaching 
  • Manage anticoagulant therapy for your own panel of ambulatory patients
  • Work with team of specialists in antithrombotic therapy and multidisciplinary physicians
  • Anticoag Team: four pharmacists, six ARNPs/PAs, three MAs, three RNs, two Receptionists
  • Implement, monitor and adjust antithrombotic pharmacotherapy for achievement of desired therapeutic outcomes
  • Provide patient education for initiation of anticoagulant therapy including bridge therapy, and management of drug interactions and adverse outcomes
  • Participate in managing transitions of care
  • Assist with quality improvement projects to improve processes related to patient care
  • Reports to Ambulatory Pharmacy Manager (PharmD, CACP)


  • Rate for a pharmacist just completing a PGY1: $54/hour
  • Rate for a pharmacist just completing a PGY2: $56/hour
  • Relocation Package, Benefits (Medical, Dental, Vision), 401K and always 100 percent vested
  • Onsite interview expenses paid
