This form is to be used AFTER a room reservation request is submitted and confirmed by the appropriate department contact.

  • Fields marked with an * are required to complete the form submission.
  • Although the Event Categorization is not marked as a required field, it must be completed with the selection “Calendar Event” so that the event will be displayed on the appropriate calendar (Public, Private, or both).
  • Events will be forwarded to Calendar administrators for review and approval.
  • You will receive an email notification when your request has been processed and another when the request has been approved or denied.
  • For modifications to events already submitted (changes in date, time, room, description, etc.), email Debbie Brown. Please DO NOT resubmit the event using this form.
