C. Immunizations & Screenings

1. College of Pharmacy Immunization Requirements and APPE Experiential Education Immunizations and Screening Requirements
   1. The College of Pharmacy Requires the following immunizations and screenings after admission
      1. Tuberculosis Testing per SWOSU policy
      2. Tetanus, Diphtheria & Pertussis vaccination
      3. Measles/Mumps/Rubella vaccination (two doses required or acceptable titer)
      4. Hepatitis B vaccination (series of three doses or acceptable titer)
      5. Varicella vaccination (series of two doses or acceptable titer, personal or physician History of Disease not accepted)
2. IPPE Rotation Immunization Requirements and Deadlines
   1. Many of the immunization/disease testing requirements for IPPE activities are fulfilled as a part of the requirements for admission into the College of Pharmacy. 
      The College of Pharmacy requires the following immunization and screenings to be completed or in process before the start of the IPPE rotations:
      1. Hepatitis B
         1. Documented evidence of 3-dose series completed (or evidence of positive titer)
      2. Measles/Mumps/Rubella (MMR)
         1. Documented evidence of 2 dose series completed (or evidence of positive titer)
      3.  Varicella/Chickenpox
         1. Documented evidence of 2 dose series completed (or evidence of positive titer)
   2. Students who are assigned to IPPE activities/courses must have the following immunizations and screenings up to date:
      1. TB Screening
         1. All students must have a TB skin test/screening/questionnaire done annually and must be valid through the student’s IPPE summer rotation. This means that the testing/screening/questionnaire would have to done during the Fall or Spring terms each year so that the screening is valid throughout the summer IPPE rotation schedule
         2. If more than 1 year has elapsed since the last testing, a two-dose series of skin testing (PPD) must be completed. Some sites may require a two- dose series done regardless of time elapsed since last testing
         3. Students may opt to complete one TB blood test (IGRA) (T-Spot, Quantiferon Gold, ) in place of the two-dose skin test. This would be required annually and must be valid through the student’s IPPE summer rotation
         4. Students with a history of a positive TB skin test, must provide Student Health Services a copy of the positive test results and history of treatment, if any was In addition, the student must have on file with Student Health Services:
            1. A chest X-Ray that will be less than two years old at the completion of the students IPPE rotations AND
            2. A current completed TB health survey (completed during the year which the rotation will be done)
      2. Tetanus/Diphtheria/Pertussis (Tdap)
         1. Documented evidence of 1 dose of Tdap administered
         2. The Td component of this vaccine must be repeated every 10 years after the childhood series (may be given as Tdap) and be valid through the student’s IPPE summer rotation
      3. Influenza Immunization (IIV or LAIV)
         1. Influenza vaccination must be current for the influenza season (October- March)
      4. COVID-19 Vaccination
         1. Proof of COVID-19 Vaccination (per CDC recommendations for health care workers), including boosters if required for up-to-date status for health care workers
            
            All records must be kept on file at Student Health Services. Email Shot Records in a PDF format.
            
            Any student who fails meet the immunization requirements or deadlines set by the Office of Experiential Education will receive a consequence determined by College of Pharmacy Administrative and Experiential Education personnel. The most severe would be a delay of progression in the program by one semester (See specific consequences outlined in the students section).
3. APPE Rotation Requirements and Deadlines
   In addition to the University immunizations and screening requirements, before starting APPPE Experiential Rotations, the following screening and immunizations must be current through the rotation time frames and meet the deadlines set by the Office of Experiential Education (OEE).
   1. The following screenings and immunizations are required for APPE rotations and must meet the deadlines set by the Office of Experiential Education (OEE).
      1. TB Screening
         1. Students beginning rotations in the Fall Semester must have two-step TB skin test or one TB Blood The second TB skin test must be placed in June. If a student elects to have a TB blood test it must be drawn in June
         2. Students beginning rotations in the Spring Semester must have a two- step TB skin test or a TB Blood test. The second TB skin test must be placed in If a student elects to have a TB blood test it must be drawn in December
         3. Students with a history of a positive TB skin test must provide Student Health Services a copy of the positive test results and history of treatment, if any was required, and have on file:
            1. A chest X-Ray that will be less than two years old at the completion of the students APPE rotations
            2. Current completed TB health survey
      2. Tetanus/Diphtheria/Pertussis (Tdap)
         1. Documented evidence of 1 dose of Tdap
         2. The Td component of this vaccine must be repeated every 10 years after the childhood series (May be given as Tdap) and be valid through the end of the student’s APPE rotations.
      3. Influenza Immunization
         1. All students starting rotations in the Spring semester must have an influenza vaccine for the current influenza season by the date provided by the OEE. Students must maintain written documentation of the immunization to provide to sites if requested
         2. Students who will be on their second semester of rotations during the Spring semester must obtain an influenza vaccine for the current influenza season by the last day of Students must update their records with Student Health, in ELMS and maintain documentation and provide to sites if requested
         3. Students must meet a site’s influenza policy if it differs from the SWOSU rotation influenza immunization Check under site and preceptor information
      4. COVID-19 Vaccination
         1. Proof of COVID-19 primary immunization series, per CDC recommendations for healthcare workers
         2. Students should be up to date with COVID immunizations and boosters per CDC recommendations for healthcare workers. (Some sites require boosters, check site and preceptor information)
         3. If students choose to receive a booster vaccination, it should be uploaded into ELMS and submitted to Student Health within 7 days of receiving the booster
         4. Students must meet a site’s COVID immunization policy if it differs from the SWOSU rotation COVID immunization policy
      5. Cumulative Record
         1. A current cumulative record must be uploaded into ELMS no later than the third working day of the month following the June / December deadline
            
            All records must be kept on file at Student Health Services. Email Shot Records in a PDF format.
